The Advocacy and Food Pantry program has completed 21 ERAP applications and identified six (6) clients for the case management pilot.
Completion of the assessment has been delayed because we have begun discussions with the Mount Sinai Health System on a larger SDH project that will standardize data collection for the purposes of billing insurance providers for providing the kinds of services that we provide.
Compiled various social determinants of health (SDH)assessments from the NYSDOH, Health+Hospitals, and other projects that LSA have participated in.
